Mary has the keys to health
Meet Mary, a 63-year old woman with significant mental health concerns as well as hypertension, diabetes, and coronary artery disease who was referred to Mental Health Access Point’s (MHAP) Keys to Health program due to multiple emergency room (ER) and inpatient admissions. Mary has been working diligently with the Keys to Health staff for a year and a half.
Shortly after enrolling with Keys to Health, Mary was linked to a primary care physician and referred to home health services. Prior to enrollment with Keys to Health, Mary had four ER contacts during the three months prior to referral. Since her enrollment with Keys to Health, Mary reduced her dependency on the ER by 50%. The assigned health coach meets weekly with Mary to monitor her status, as well as accompany her to medical appointments and helps her follow treatment recommendations. The Keys to Health care coordinator collaborates with the client, family, and the health coach to identify any needs and make appropriate referrals.
